The Service Planning and Rehabilitation Specialist works with the Quality Management Department to provide the highest achievable quality of services to the individuals we serve. The Quality Management Department defines best practices, identifies strengths in service delivery and provides structure through which problem identification and resolution can occur. Service Planning and Rehabilitation Specialists at DePaul will provide oversight of service planning and Medicaid documentation within OMH certified residential program to include development, implementation, training and auditing.

Quality Management and Related Activities:
Responsible for regular Medicaid Case Record Reviews on all resident’s files in the OMH certified programs of at least 10% of the total number of files monthly. This includes reviewing Physician’s Authorizations, admission notes, service plans, service plan reviews, and progress notes.
Responsible for the review of progress notes, especially rehabilitation service documentation, as it pertains to billing compliance.
Responsible for completing discharge verification process for residents discharged from OMH licensed CR and TAP residences.

Clinical Consultation:
At the request of the Director of QM, perform case-related clinical consultation including, but not limited to; Case Reviews, Treatment Team Meetings, Risk Management Meetings and Crisis Intervention.

Education & Experience:
Bachelor’s degree in Mental Health field or closely related field. QMHS status required. Three years of experience in Mental Health required. Five years of mental health related experience preferred. Must have the ability to work well with others in a challenging setting and establish appropriate boundaries necessary for a professional work environment.
*In addition to the above criteria this position also requires a clean New York State driving record as outlined in DePaul’s personnel policy and the use of one’s personal vehicle is also a requirement.
