Wheatfield Commons, a DePaul Senior Living Community is hiring a full - time Memory Care Coordinator for the day shift.

 

The pay range for this role is $23.00 - $23.60

Responsibilities

Responsibilities

 

1.   Respects and maintains resident rights and confidentiality and demonstrates a caring, positive attitude toward all residents, staff, guests, and service providers.

2.   Performs the duties and responsibilities of the Medication Coordinator as assigned.

3.   Performs the duties and responsibilities of the Supervisor as assigned.

4.   Assists in recruiting, screening, and interviewing resident care staff. Assists with the orientation for new resident care employees.

5.   Assists in conducting in-service education and training programs for staff on a monthly basis.

6.   Attends and participates in staff and team meetings.

7.   Makes recommendations for assignments and promotions. Participates in coaching, corrective, and disciplinary personnel plans.

8.   Assists with the development of all staffing schedules for the Resident / Memory Care Department.

9.   Completes rounds daily, make recommendations for daily resident care assignments and ensures follow-through of resident care staff. Provides assistance to resident care staff when necessary.

10.  Assists the Resident Care Director to ensure that all policies, procedures, and Department of Health regulations are adhered to in the department.

11.  Consults with the Resident Care Director as needed concerning resident care and personnel concerns.

12.  Assists with the admission, transfer and discharge of residents as needed.

13. Performs other duties as needed or requested by the Director and / or Administrator.

 

Essential Functions

  1. Ability to communicate with residents and staff in English.
  2. Ability to read and write English.
  3. Ability to lift up to 30 pounds unassisted.
  4. Ability to visually observe residents, to hear emergency systems and verbal needs expressed by residents.
  5. Ability to turn, stoop, bend and stretch in order to assist residents.
  6. Ability to stand for prolonged periods.
  7. Ability to use equipment competently.
  8. Ability to respond to emergencies, i.e. resident choking, evacuation of residents and visitors in the event of fire.
  9. Ability to push medication cart down hallway without assistance.

 

Qualifications

  1. Must be at least 18 years of age.
  2. High School Diploma or GED
  3. Three years’ experience as a supervisor, working with the elderly, preferably with those that have dementia diagnosis.
  4. Well developed organizational skills.
  5. Ability to work effectively with inspectors, monitoring, and licensing agencies.
  6. Must have the required annual Tuberculosis (TB) testing and annual Health Assessment completed.
  7. Must have the required references and criminal record check.
  8. Must have a valid New York State driver’s license that meets agency’s clean driving record policy.
